Referring physicians must first register and log in to the Online Referral System in order to refer their patient(s) to one of the OBN Bariatric Programs. For more information on these programs and bariatric treatment options available, please click here.

Please note that referring physicians accessing the new online referral system for the first time must register to create a new account; username and log-in credentials from the old Bariatric Registry website are not accepted.

As of April 1, 2015, the Ontario Bariatric Network in conjunction with the Ministry of Health and Long Term care has revised the Ontario Bariatric Program referral process. All referrals must now be submitted using the new online referral system by clicking here  

How the NEW online referral system works:  

  • Primary care physicians (PCP) register only once  
  • The system has built in logic which utilizes key eligibility questions to route patients 
    immediately to the appropriate OHIP funded bariatric program at one of our Ontario Bariatric Centres  

Why the NEW online referral system was implemented:  

  • Our program demand is growing and referrals need to be centralized and processed within 24hrs  
  • The fax based system was no longer effective at meeting the needs of our provincial bariatric program which has diversified into three streams:  
  • Medical Program 
  • Surgical Program 
  • Metabolic Pilot Program  

The NEW online referral system has enhanced user capability:  

  • Program eligibility and bariatric centre location is confirmed at time of referral  

Physicians will be able to:  

  • Track patient status and receive timely updates  
  • Sort and filter their patient referral list  
  • Set email notification preferences  
  • Print or save the referral form  

STEP 1: REGISTER FOR AN ACCOUNT 

  • Health Care Providers register by clicking here 
  • To register you will be asked for some basic information: 
  • OHIP billing number 
  • Contact information 

STEP 2: LOGIN 

  • Once registered, you will now have a login and password 
  • You can use your OHIP billing # to login 

STEP 3: SUBMIT A REFERRAL 

  • Select “submit referral” 
  • Based on the information entered, eligibility will be confirmed – your patient may be eligible for none, one, or two of the Bariatric Programs offered 
  • Enter your patients contact information 
  • Centre allocation will be shown 

STEP 4: OUTCOME 

  • Once submitted the referral will be immediately forwarded for follow-up 
  • The centre to which your patient has been allocated will contact your patient directly and invite them to an orientation session 
  • You can print or save the referral forms for your records 

STEP 5: TRACK PATIENT PROGRESS 

  • Using the “My Referrals” tab you can now track your patients’ progress through the bariatric program 
  • The referral system is linked to the centres case management system, so you will be notified when your patient is booked for orientation

  1. Using Google Chrome or IE 9 (or later); navigate to the OBN referrals site 
  2. Select “Register here” from the Bariatric Referral landing page
  3. Select your role from the “I am a…” drop down list
  4. Enter your title (optional), first name, last name and email
  5. Complete the “My Details” section of the registration form and select “Register”
  6. Select “Yes” to confirm
  7. The following message will be displayed; select “Ok”
  8. You will receive an email from the Bariatric Referral System with your unique username and temporary password

  1. Using Google Chrome or IE9 (or later); navigate to https://referrals.ontariobariatricnetwork.ca
  2. Select “Register here” from the Bariatric Referral landing page
  3. Select your role from the “I am a…” drop down list
  4. Enter your title (optional), first name, last name and email
  5. Complete the “Physicians Details” section of the registration form and select “Register” (Note: You can add multiple Physicians by selecting “Add another Physician”)
  6. Select “Yes” to confirm
  7. The following message will be displayed; select “Ok”
  8. You and the Physician(s) will receive an email from the Bariatric Referral System with your unique username and temporary password
  9. Once the email is received, return to https://referrals.ontariobariatricnetwork.caEnter your username and temporary password and select “Sign In” 
  10. The system will display a message that your password has expired; Select “Ok”
  11. Enter your old (temporary) password and new password and select “Change Password”

PLEASE NOTE: Passwords should have a minimum of 8 characters and contain a mix of character types (lower case, upper case, numbers and/or special characters – such as !/@/#/$)

Project ECHO OBN is a virtual interactive education and capacity building program that supports health care providers in delivering quality care to their patients living with obesity or who have received specialty care from a Bariatric Centre of Excellence.

Sessions are open to primary care providers and allied health professionals working in primary care. There is no cost to participate.
